Does an adverse medical outcome automatically establish negligence+

No. The assessment must examine the standard of care, whether it was breached, whether that breach caused the alleged harm and what loss resulted.

Which records are useful for an initial assessment+

Discharge letters, test results, imaging, operative notes, consent forms, prescriptions and a clear chronology are usually helpful. A complete record may be requested if important documents are missing.

Are there deadlines for medical-liability claims+

Yes. Different civil, criminal, administrative or disciplinary routes may have different deadlines. Prompt advice is important, particularly where treatment occurred some time ago.
